  1. There are changes for the 2023 Registration & Payment process.  Please read carefully.

    -      All registration will be completed online between Feb 1 and March 31, 2023.
    -      All Coaches and helpers need to complete a Coaches Application found under the registration tab on our website (Deadline Feb 28, 2023)
    -      All House League Registration will be a “first come first serve basis” & will be time stamped from your online submission.  Each coach will determine the number of players they will carry on their team.  Once a team is full players will be placed on a wait list for a second team.
    -     Select teams be based on coaches decisions during the tryouts

    Registration Fees for 2023
    Midget 19U          (2005-2007)   $175  
    Bantam 15U        (2008-2009)   $175  
    Pee Wee 13U        (2010-2011)   $130  
    Mosquito 11U        (2012-2013)   $130  
    Rookie 9U          (2014-2015)   $130  
    JR. Rookie 7U      (2016-2017)   $130  
    Tee Ball   (2018-June 2019)   $50  (Must be 4 years old on June 15)

    Volunteer /Raffle Tickets ($50 postdated cheque per family) - In Order to help keep the cost of baseball low we will be running a raffle again this year.  The 2023 family volunteer / raffle requirement will work as follows.

    -       Each family submits a $50 volunteer cheque with registration postdated July 19, 2023
    -       Families that agree to volunteer to sell tickets will be given a booklet of 10 tickets worth $5 each
    -       Tickets will be handed out by parent reps during the season
    -       Families will sell the tickets and return the stubs with the $50 ticket proceeds to their parent rep
    -       Once tickets are returned with the $50 proceeds FMBA will return/destroy the volunteer cheque
    -      If tickets and money are not returned by the due date or a family declines to volunteer to sell the tickets the post dated cheque will be cashed

    Coaches
    We will need at minimum 1 coach and 1 assistant coach for each group of kids.  If you are able to help please complete the Coaches / Helper signup.  All coaches and helpers will be required to submit a current Vulnerable Sector police check.

    Equipment Requirements
    All players will be required to provide their own Glove, Batting helmet and Water bottle.  Player are also recommend to provide there own bat at U11 and up. 

    Uniform deposit -  ** SEE CHANGES FOR 2023 ** 
    UFMBA will require a $100 posted dated cheque dated August 23, 2023.  Upon the return of the uniform and attendance at the annual parent meeting on August 23, 2023 the cheque will be returned.    If a parent is unable to attend the parent meeting on Aug 23, 2023 the cheque will be cashed after the meeting and you will still be required to return the jersey as it is the property of FMBA. 


    Select Team Fee
    - All players playing on a Select Team will be required to pay an additional $75 select team fee to cover the cost of league registration.  If you child is trying out for select please bring the $75 cheque to the first practice.  This check will only be cashed if the player makes the team.  


    NSF Cheque Fee
      -$25.00 payable to FMBA

    Refunds  - Will be given until May 31st, 2023 less a $30.00 Administration Fee.  After May 31st, Registration Fee and Raffle Ticket Fee WILL NOT be refunded. Post dated Uniform Cheque will be returned when the players uniform is turned in. 


    Payment by CHEQUE due by March 31, 2023
    Each family will be required to submit the following cheques made out to Forest Minor Baseball Association by March 31, 2023

    Cheque 1 - Registration fees (see chart)

    Cheque 2 – Select fees if applicable ($75)

    Cheque 3 – Volunteer $50 (per Family post dated July 19, 2023)

    Cheque 4 - Uniform deposit (Post dated Aug 23, 2023)

     

    Late payments will drop the player to the bottom of the wait list.  Cheques will only be processed if there is a season

  1. Dual Registration

    By completing this form, you are registering with both FMBA and Baseball Ontario. Baseball Ontario appoints FMBA to collect fees on its behalf, included in the registration fee collected through this process. No additional monies is charged to the parent registering locally. Baseball Ontario invoices FMBA for fees collected.

  2. Liability Waiver

    On behalf of myself and the player being registered, I recognize that baseball entails serious risks. Consequently, except as noted below, I and the player being registered relinquish all rights to a claim of any kind, including the right to a claim for bodily and material damages, regardless of the cause, against Baseball Ontario, its member associations, including FMBA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors, even if such damages result from negligence of Baseball Ontario, its member associations, including FMBA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors.

    Without restricting the generality of the preceding, I and the player being registered also relinquish the right to any claim against Baseball Ontario, its member associations, including FMBA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of the nature of this decision.

    This release of Baseball Ontario, FMBA and the other persons noted above does not preclude myself or the player being registered from making a claim under any sports accident coverage provided by Baseball Ontario and/or FMBA to their players.   3. Photo Consent

    I authorize FMBA and/or their respective staff, associates, assistants, or subcontractors to use photograph(s), video and other media image provided for any local association promotional, educational or other pertinent uses. I also agree to FMBA using any images of the named minor(s) gathered by photographers, and or videographers supplying images to my FMBA. These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions by various member associations of my local association. I authorize FMBA to permit the use and display of photographs and/or recordings of the named minors in any FMBA publication, multimedia production, including video and web usage, display, or advertisement. I agree that FMBA may use name, likeness, or information supplied by the undersigned. The undersigned releases and forever discharges FMBA and their respective officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.

  5. Rowan’s Law

    Under Rowan's Law, before any player can be registered with their local association and Baseball Ontario, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct.
